Instructions:

• First, enter your name and date of birth (mandatory fields).
• You can optionally add your NHS number.
• Then, enter your Systolic (top number) and Diastolic (bottom number) blood pressure readings.
• Select the date and time for each reading.
• Click “Add Reading” to add more entries.
• Once all readings for your 4 or 7-day period are entered, click “Submit All Readings”.
